Transliteration

Allahumma salli `ala Muhammadin wa `ala ali Muhammadin kama sallayta `ala Ibrahima wa `ala ali Ibrahima innaka hamidun majid. Allahumma barik `ala Muhammadin wa `ala ali Muhammadin kama barakta `ala Ibrahima wa `ala ali Ibrahima innaka hamidun majid.

Meaning

O Allah, send Your grace on Muhammad and on the family of Muhammad, as You sent grace on Ibrahim and on the family of Ibrahim. You are indeed Praiseworthy, Most Glorious. O Allah, send blessings on Muhammad and on the family of Muhammad, as You sent blessings on Ibrahim and on the family of Ibrahim. You are indeed Praiseworthy, Most Glorious.

Where you already say it

At every tashahhud, after Ashhadu an la ilaha illa Allah wa ashhadu anna Muhammadan `abduhu wa rasuluh. It comes before the concluding salam. This is the classical Hanafi, Shafi`i, Maliki and Hanbali position, with minor wording variants.

Why the Prophet taught this specifically

The companions asked: how do we send blessings on you? He taught them this exact form (Sahih al-Bukhari 3370). Everything else is derivative. If you memorise one salawat in your life, this is the one.
